Position Summary
Make a Difference at Phoenix Children’s! Phoenix Children’s is seeking a compassionate and dedicated Social Worker to join our Palliative Care team. In this role, you will provide support, advocacy, and care coordination for patients and families navigating complex medical needs and care transitions.

Participates as an interdisciplinary team member providing interventions with the patient/family that include a range of care from bio-psychosocial assessment, diagnosis, treatment planning, therapeutic interventions, crisis interventions, coordination of care, bereavement support, and transition/discharge planning. The position requires knowledge of family systems and interpersonal dynamics/relationships and is able to use that knowledge to assist patients and families in achieving therapeutic goals. Knowledge of mandated reporter responsibilities; use of financial, community and other resource networks; coordination with DCS, Police jurisdictions, and other governmental authorities; ability to advocate with outside systems; and, collaborate with medical staff. The position may provide individual, group, family therapeutic interventions for an assigned caseload. The position is responsible for implementation of patient treatment assignments, management of open patient records, and for collaborating with outside referral sources as applicable.
Education
- Masters in Social work at an institution accredited by the Council of Social Work Education or an equivalent foreign degree as determined by the Foreign Equivalency Determination Service of the Council on Social Work Education. (Required)
Experience
- Child protection experience. (Preferred)
- Three years post-Masters social work experience in a health or behavioral health care setting. (Preferred)
- Pediatric social work experience. (Preferred)
Certifications
- Current State of Arizona Licensed Master in Social Work by the Arizona Board of Behavioral Health Examiners. (Required)
- CPR/First Aid and CPI training or the ability to obtain at hire. (Preferred)
- Valid AZ Department of Public Safety (DPS) Fingerprint Clearance Card (Required)

- Excellent communication skills, both verbal and written (Required)
- Effective decision-making, problem solving skills, demonstration of creativity in problem solving, patient and family strengths, influential leadership skill. (Required)
- Critical thinking skills and ability to anticipate patient discharge needs. (Required)
- Ability to assess symptoms and develop diagnostic impressions and treatment plans. (Required)
- Moderate to expert computer skills. (Required)
