St. Leonard is seeking a Registered Nurse (RN) with RAC-CT certification and long-term care experience to join our nursing team as an MDS CoordinatorIn this role, youll coordinate Minimum Data Set (MDS) resident assessments, support individualized care planning, and help ensure accurate clinical documentation and regulatory compliance.
